All requested information must be provided unless explicitly indicated as optional.

Incomplete forms will be returned.

Please note, root level domain names such as www.jhmi.edu or med.jhmi.edu require justification and are not given out to normal hosts. Servers which are used by the entire Hopkins community, and not just a specific group, are good candidates for root level names.  Please answer all questions completely and allow extra time for this type of request to be processed. You may be contacted during the review of your special request for additional justification or information.

To expedite your request, if you are requesting a hostname on the jhmi.edu domain, please ask your local LAN Administrator for a specific IP address before completing this form.

LEGAL NOTICE:
1. In no event shall JHU or its affiliates be liable for any direct, indirect, incidental, special, punitive, reliance or consequential damages of any kind, including, but not limited to, loss of business, profits, or information or other commercial or economic loss, whether such damages are based upon breach of contract, tort, strict liability or otherwise, even if JHU have been advised of such damages and regardless of whether the damages were foreseeable.
2. We reserve the right to change, modify or interrupt your specific connectivity to maintain network integrity at our discretion. JHU will attempt, but has no obligation, to provide advance explanations and/or warning of the changes to the service.
3. Participant assumes any and all risk of loss, damage or liability which Participant may sustain while using the JHU Network or JHU related resources, services and facilities and Participant hereby releases and agrees not to sue JHU, officers, agents, employees and/or students, for any such loss, damage or liability in connection therewith.
4. In the event of an emergency and/or to protect JHU, the JHU Network, other JHU participants or other persons or entities, JHU may, without prior notice, modify, suspend, or discontinue any aspect of the JHU Network operations, impose limits on JHU features and operations, or restrict any Participant's access to any part or all of the JHU Network. As soon as feasible thereafter, JHU will attempt to contact Participant in an attempt to resolve any problems and reestablish service within a reasonable time period.
5. Either party may terminate this agreement prior to the expiration of its then current term by providing 30 days written notice.
